1. I/We hereby request:
a. Glistian (Pty) Ltd, or its nominee, to draw against my / our account at whichever bank it may be at present, the
minimum amount stipulated in the selection of services rendered, being the first amount due on my account in respect of services provided purchased by me and / or any increases thereof and / or outstanding amounts on
my account, due in respect of the agreement dated upon signature of this debit order authorization form. b. My/our bank, whichever it is or will be, to debit my / our account with such amounts drawn against it by Glistian (Pty)
Ltd, or its nominee, in terms of this request.
Please debit my Bank Account indicated below, the first payment due upon subscribing for services provided by Glistian (Pty) and the date of signature hereof, and then on a monthly basis
thereafter.
2. I/we understand and agree:
a. That the withdrawals from my/our bank account will be processed through a computerised system provided by the South
African Banks and I/we also understand that details of each withdrawal will be printed on my/our bank statement or on
any accompanying voucher.
b. To pay any bank charges to the Debit Order Instruction. In the event of the debit order being rejected twice by the bank, I/we
note the special notice of Glistian (Pty) Ltd, or its nominees, obligation to cancel this instruction and hereby give
them permission thereto.
c. That I/we shall not be entitled to any refund of amounts, which have been withdrawn while this authority was in force if such
amounts were legally owed.
d. That although this Authority and Mandate may be cancelled by me/us by giving 30 days’ notice in writing and sent by registered
post or by email, such cancellation will not cancel the original Agreement.
e. That this authority may be ceded or assigned to a third party if the Agreement or the management of the Agreement is also
ceded or assigned to that third party, but in the absence of such assignment of the Agreement or management of the
Agreement, this Authority and Mandate may not be assigned to any third party.
3. The Client consents to and authorises Glistian (Pty) Ltd, or its nominee to:
a. contact, request and obtain any information at any time and from any credit provider (or potential credit provider) or registered
credit bureau in order to assess the behaviour, profile, payment patterns, indebtedness, whereabouts, and creditworthiness
of the Client; and
b. provide any information about the behaviour, profile, payment patterns, indebtedness, whereabouts, and creditworthiness of
the Client to any registered credit bureau or to any credit provider (or potential credit provider) seeking a trade reference
regarding the Client’s dealings with Glistian (Pty) Ltd, or its nominee.
